This form is only for the measured-voice pages: a reading you think is wrong, or a request from the person (or someone with standing to act for them) to take a page down.

We are specific about both: if a reading is wrong, we re-measure it and correct the page; if someone with standing asks for a voice to come down, it comes down. Every measured page documents its source recording and method for exactly this reason.
